How Mesothelioma Is Treated
No two mesothelioma treatment plans are identical. The right approach depends on a combination of factors, including the stage at diagnosis, tumor location, cell type, and the patient’s age and overall health.
Curative vs. Palliative Treatment
Treatment can be pursued with two different goals. For patients diagnosed at an earlier stage who are in good overall health, the aim is to remove or destroy as much of the cancer as possible. For patients with more advanced disease, the focus shifts toward controlling tumor growth, relieving symptoms, and preserving quality of life.
The Importance of Specialist Care
Because mesothelioma behaves differently from more common cancers, treatment decisions are best made by specialists with direct experience managing the disease. A general oncologist seeing their first mesothelioma patient will have significantly less context than a thoracic surgeon or oncologist who treats it regularly.
Seeking care at a cancer center with a dedicated mesothelioma program and getting a second opinion before committing to a treatment plan can open doors to options that might not otherwise be available.
Standard Treatment Options
Mesothelioma is treated using several established approaches, often in combination. The options available to a patient depend on their diagnosis, stage, and overall health.
- Surgery removes tumors and affected tissue, and is most effective in earlier-stage disease.
- Chemotherapy uses drugs to kill cancer cells or slow their growth.
- Immunotherapy helps the body’s immune system identify and attack cancer cells.
- Radiation therapy uses high-energy rays to target and shrink tumors.
- Multimodal therapy combines two or more of the above approaches for a more comprehensive treatment effect.
Surgery
Surgery is the most aggressive treatment option for mesothelioma and is recommended for a specific subset of patients: primarily those with early-stage, epithelioid disease who have limited tumor spread and are treated at high-volume centers with mesothelioma surgical expertise. Current guidelines call for careful patient selection and specify that surgery should not be offered solely because it is anatomically feasible.
For pleural mesothelioma, the two main surgical procedures are pleurectomy/decortication (P/D), which removes the pleural lining and visible tumors while preserving the lung, and extrapleural pneumonectomy (EPP), a more extensive procedure that removes the entire affected lung along with the pleural lining, part of the diaphragm, and the pericardium. P/D is generally preferred where possible. EPP is less commonly recommended under current guidelines.
For peritoneal mesothelioma, cytoreductive surgery removes tumors from the abdominal lining and is typically combined with Hyperthermic Intraperitoneal Chemotherapy (HIPEC) delivered directly into the abdominal cavity during the procedure.
For pericardial mesothelioma, pericardiectomy removes part or all of the pericardial lining surrounding the heart. For testicular mesothelioma, radical orchiectomy removes the affected testicle. Because both types are so rare, no formal treatment guidelines exist for either, and surgical decisions are made on a case-by-case basis with a specialist.
Chemotherapy
Chemotherapy uses drugs to kill cancer cells or slow their growth and is one of the most commonly used treatments across all mesothelioma types. It can be used as a standalone treatment, before surgery to shrink tumors, after surgery to reduce the risk of recurrence, or in combination with immunotherapy.
First-line systemic treatment options for pleural mesothelioma include three approaches:
- Chemotherapy alone (pemetrexed with cisplatin or carboplatin)
- Doublet immunotherapy (nivolumab and ipilimumab)
- Chemoimmunotherapy (pembrolizumab with pemetrexed and platinum-based chemotherapy).
For patients with non-epithelioid disease, current guidelines recommend immunotherapy-based regimens as the preferred first-line approach. Chemotherapy alone should generally not be offered to patients with sarcomatoid or biphasic disease unless there is a medical reason that immunotherapy cannot be used. For epithelioid disease, all three options are reasonable, and the choice is individualized based on the patient’s health, kidney function, and treatment goals.
For peritoneal mesothelioma, chemotherapy is most commonly delivered as part of HIPEC during cytoreductive surgery, using cisplatin or doxorubicin heated and circulated directly through the abdominal cavity. Systemic chemotherapy with pemetrexed and cisplatin or carboplatin is also used for patients with peritoneal cancer who are not candidates for surgery.
Immunotherapy
Immunotherapy works by helping the body’s own immune system recognize and attack cancer cells. It has become an increasingly important part of mesothelioma treatment, particularly for pleural mesothelioma, following a series of significant regulatory approvals in recent years.
In 2020, the FDA approved the combination of nivolumab (Opdivo) and ipilimumab (Yervoy) as a first-line treatment for unresectable pleural mesothelioma. This was followed in 2024 by FDA approval of pembrolizumab (Keytruda) in combination with pemetrexed and platinum-based chemotherapy as a first-line treatment for pleural mesothelioma. Both approvals marked a meaningful shift in how the disease is treated, giving newly diagnosed patients access to therapies that had not previously been available as standard options.
Immunotherapy is not yet FDA-approved as a first-line treatment for peritoneal, pericardial, or testicular mesothelioma, though some specialists use it in these patients based on emerging evidence.
Radiation Therapy
Radiation therapy uses high-energy beams to target and destroy cancer cells. For mesothelioma, it is rarely used as a standalone treatment but plays an important role in multimodal treatment plans.
For pleural mesothelioma, radiation is most commonly used after surgery to target any remaining cancer cells and reduce the risk of local recurrence. Intensity-modulated radiation therapy (IMRT) is the most commonly used technique in this setting. Radiation is also used palliatively to relieve pain caused by tumor growth or chest wall involvement.
Radiation is less commonly used for other mesothelioma types, and its role varies depending on tumor location and the overall treatment plan.
Multimodal Therapy
Multimodal therapy combines two or more treatment approaches and is widely regarded as the most effective strategy for eligible mesothelioma patients. For pleural mesothelioma, this typically involves a combination of surgery, chemotherapy, and in some cases radiation therapy, sequenced based on the patient’s individual treatment plan. For peritoneal mesothelioma, cytoreductive surgery and HIPEC together form the primary multimodal approach.
Eligibility depends on disease stage, overall health, and the ability to tolerate multiple treatments. For patients who are not surgical candidates, chemotherapy and immunotherapy combinations serve as the primary multimodal option.

Palliative Care
Palliative care focuses on relieving symptoms and improving quality of life rather than treating the cancer itself. It is not a last resort and can be integrated alongside active treatment at any stage of the disease.
Common palliative interventions include procedures to drain or prevent fluid buildup around the lungs or abdomen, such as thoracentesis, paracentesis, and pleurodesis. Pain management, nutritional support, and psychological care are equally important components of a palliative approach.
Clinical Trials and Emerging Treatments
Clinical trials give patients access to treatments that are not yet widely available, including newer immunotherapy combinations, targeted therapies, and CAR-T cell therapy. Tumor treating fields, a therapy that uses alternating electric fields to disrupt cancer cell division, has received FDA approval in combination with chemotherapy for unresectable pleural mesothelioma and represents one example of how the treatment landscape continues to evolve.
For patients who have exhausted standard options or are at a stage where standard treatment has limited impact, clinical trials represent a meaningful pathway. Enrollment does not mean giving up on treatment. Many trials test whether a new approach outperforms the current standard, and some patients achieve outcomes that exceed what standard care would have offered.
